علاج اضطرابات ليله الذفاف

ليلة الزفاف في الدول الشرقية ذات التقاليد المحافظة تعتبر مناسبة خاصة جدا ينتظرها الشباب لسنوات طويلة وتعتبر من أجمل ذكريات العمر لأغلب الأزواج والزوجات ، ولكن عند بعض الشباب الذي يعاني من فقدان الثقة والخوف من الفشل، تصبح ليلة الزفاف امتحان إما أن ينجح أو يتعرض لحرج شديد.

في مثل هذه الحالات مع الخوف من الإقدام على الزواج أو حتى في حالات الفشل في بداية الزواج يستطيع الطبيب المعالج أن يصل إلى تشخيص المرض عن طريق التاريخ الطبي الدقيق والفحص الإكلنكي وبعض الاختبارات إن لزمت لبناء خطة علمية للعلاج.

أغلب هذه المتاعب يرجع إلى فقدان الثقة في الانتصاب ويقع المريض في حلقة مفرغة من الخوف من الفشل الذي يؤدي إلى تكرار الفشل ولذلك يكون الهدف الرئيسي للعلاج هو كسر هذا الحاجز النفسي والنجاح في المعاشرة الزوجية والتخلص من عدم الثقة.

في الماضى كان علاج عدم الثقة في الانتصاب هو عن طريق بعض التدريبات، يؤديها الزوجان معا "وهي تدريبات ماسترز وجونسون"، ولكن في حالات كثيرة يجد الأزواج حرجا في تأدية تدريبات وصفها لهم الطبيب خاصة في بداية زواجهم, كما كانت فرص النجاح ليست مرتفعه ولذلك وجب البحث عن حلول بديلة. وهنا يجب مراعاة أن الكثير من حالات الفشل ترجع إلى خوف الزوجة وانقباض عضلات الحوض لا إراديا مما يمنع الإيلاج.

يستجيب بعض المرضى للأدوية المنشطة التي تستخدم لفترات مؤقتة مثل دواء السيالس او الفياجرا وقد اجرى الباحثون بكليه طب القصر العينى بحثا تم نشره فى المجله الدوليه لابحاث الضعف الجنسى ووجدوا ان استعمال عقار السيالس او الفياجرا ادى الى شفاء المرضى با اكثر من 91% من الحالات

في حالة إذا كان الضعف يرجع إلى سبب عضوي، كخلل في الدورة الدموية أو الأعصاب أو الهرمونات يكون العلاج موجه للسبب ويمكن أيضا استخدام الفياجرا أو الحقن الموضعي أو الجراحة لعلاج هذه الحالات، ولذلك ينصح الرجال بعدم قبول الأمر الواقع والمعاناة في صمت، إن الأغلبية الساحقة من الأسباب النفسية والعضوية يمكن علاجها عن طريق الوسائل العلاجية والتشخيصية الحديثة.

Aim: The aim of this study is to evaluate the effectiveness alternate-day tadalafil

Methods: This is a descriptive study comprised of a series of 45 patients. The time

frame for the study was 2 years. Forty five consecutive patients underwent a complete

Objective: The aim of this study is to evaluate the effectiveness of short term sildenafil use in the management of unconsummated marriages diagnosed to be mainly psychogenic in origin.

Materials and Methods: This retrospective study included 35 patients evaluated within an Andrology clinic. Patients underwent a complete medical and sexual history as well as a focused physical examination. Investigations were ordered as necessary following a goal directed approach. Education about the male and female genital anatomy and the sexual response cycle was done, as well as a detailed explanation about the concepts of performance anxiety, vaginismus and the mode of action of sildenafil. Sildenafil on demand therapy was initiated for one month and the duration extended as needed.

Results: Of 35 patients included in our study 32 (91%) were able to achieve vaginal intromission and perform sexually. Twenty three patients needed the sildenafil (66%) for less than 1 month, 5 (14%) for up to 3 months and 4 (11%) for more than 3 months. Three patients (9%) were unsuccessful. Treatment failures were managed by intracavernous injection therapy, combined with psychosexual therapy, depending on the cause.

Conclusions: We conclude Sildenafil use is effective as a short term treatment option in the management of unconsummated marriages.
