Etiology

A) With normal secondary sexual characters

B) Wth absent seconary sexual characters

C) With heterosexual develpment .

The most common causes (according to Kase) :

1. Ovarian failure whether normal or abnormal karyotype

2. Mullerian agenesis

3. AIS              4. Constitutional delay .

Normal secondary sexual characters (delayed menarche only) :

1. Imperforate hymen                          2. Transverse vaginal septum

3. Vaginal agenesis without or without functioning uterus

4. Complete type of Androgen insensitivity syndrome

5. Persistent ovary syndrome and PCO       6. Consistutional delay of menarche

With heterosexual development :

1. Ovarian : PCO , virlizing ovarian tumors

2. Adrenal : Delayed CAH, Cushing syndrome and andogen secreting tumor

3. Certain cases of male pseudohermaphroditism e.g. 5 -reductase deficiency, absent MIF and incomplete AIS .

4. True hermaphroditism

Absent secondary sexual characters (sexual infantilism) :

        

  Tall or normal       Short 
Hypogonado-                                               GnRh deficiency / Kallman syndrome Laurance Moon Biedl syndrome 
tropic    Weight loss and excercise Levi lorain syndrome
  Hyperprolactinemia   Hypothyroidism
  CNS tumor  
Hypergonado-  Gonadal agensis (XX or XY [Swyer]) Turner
tropic   Mixed gonadal dysgenesis (rarely) Mixed gonadal dysgenesis
   Ovarian failure (radio or chemotherapy or Galactose defeciency)   True hermaphroditism
