II) Peritoneal fluid environment :
· Peritoneal fluid volume ( normally reaches a peak of 20 ml at ovulation ) :
- In patients with endometriosis , the PF volume is increased .
- Patients with endometriosis and PF volume < 14 ml , has a higher pregnancy rate.
· Peritoneal fluid prostaglandins :
- Mechanism : prostaglandins increase the PF volume , alter tubal muscle contractility and are luteolytic
- Pg increased are : Pg F2a , Pg E2, TX B2 and 6-keto Pg F .
- Source of these Pg : is unknown but may be endometriotic foci or macrophages .
· Peritoneal fluid macrophages :
- Mechanism : sperm phagocytosis
- Increased activation of macrophages have been found , however patients with and without endometriosis have the same no. of motile sperms in the peritoneal fluid .
- Source : is unknown , but may be fallopian tubes or endometrium
