- Before treatment 2 basic points should be taken into consideration :
1. Screening for endometrial carcinoma should be dine
2. Reduction of weight
- Lines of treatment will depend on the presentation :
1. Anovulation and infertility : induction of ovulation .
· Dexamethazone could be added : 0.5 mg bed time to decrease night time peak of ACTH which will decrease the adrenal contribution to the androgens . Higher conception rate is documented if DHAS is high .The dose of clomiphene is reduced to 50 mg and then increased in staircase fashion .
· HMG should be used with great care
2. Hirsutism : Decrease LH + antiandrogen + local hair removal
3. DUB : cyclical progestins
4. Mangement of endometrial hyperplasia and carcinoma
