Surgery should not be resorted to except in very rare cases. These are the cases that will fail to ovulate with either CC or gonadotrophin therapy. This entitles coagulating the androgen producing tissues within the ovarian stroma. An electrically heated fine needle is introduced guided by laparoscopy to electrocauterise punctures (drilling) within the ovarian tissue. Although effective in achieving ovulation, cases that fail to achieve pregnancy may suffer peri-tubal pelvic adhesions as an aftermath of ovarian drilling. Ovarian atrophy and premature menopause are recorded in the medical literature as a rather rare complication of ovarian drilling. This treatment modality should only be offered and exercised with utmost caution and in limited conditions.
